Abstract

Background
Approximately 10% of patients report a penicillin allergy; however, up to 90% of these individuals are not truly allergic. Inaccurate penicillin allergy labels contribute to inappropriate antibiotic use, increased healthcare costs, and antimicrobial resistance. Education on penicillin allergy testing remains limited among future Family Nurse Practitioners (FNPs).

Methods
A quantitative quality improvement project using a pretest-posttest design was conducted among second- and third-year Doctor of Nursing Practice–Family Nurse Practitioner (DNP-FNP) students enrolled in Pharmacology II or Advanced Practice Nursing III at a Midwestern liberal arts college. Participants completed a 15-minute voiceover PowerPoint educational intervention on penicillin allergy testing and de-labeling. Knowledge and confidence were measured using a four-item questionnaire based on the Perceived Competence Scale. Pre- and posttest responses were analyzed using a Wilcoxon signed-rank test.

Results
Seven students completed the intervention and both surveys. Significant improvements were observed in knowledge of penicillin allergy testing (p = .026), ability to conduct testing (p = .017), and ability to manage testing challenges (p = .018).

Conclusion
A brief educational intervention significantly improved DNP-FNP students’ self-reported knowledge and confidence regarding penicillin allergy testing. Incorporating penicillin allergy education into graduate nursing curricula may help prepare future providers to implement evidence-based allergy de-labeling practices and support antimicrobial stewardship efforts.
